Seeking Case Management Discharge Specialist!
Full-time, Monday-Friday, 8:00am-4:30pm, no weekends/holiday
Baptist Health Floyd, Care Coordination
Southern Indiana/Louisville, Kentucky area
The Case Management Discharge Specialist ensures a seamless transition for patients from the hospital to home or other care settings by facilitating timely and effective post-acute follow-up appointments. This role involves collaborating with healthcare providers, patients, and families to coordinate care plans, address barriers to follow-up, and promote continuity of care. By aligning discharge processes with individual patient needs, the Discharge Specialist plays a pivotal role in optimizing recovery, reducing readmissions, and enhancing overall patient satisfaction. This position directly contributes to the organization’s mission of delivering exceptional care and achieving positive health outcomes.

Qualifications:
- High school diploma/equivalent
- 2 years of Discharge planning/Case Management e xperience
