Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health and Hospice team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. The RN will work collaboratively with a multidisciplinary team to develop and implement individualized care plans, monitor patient progress, and educate patients and their families on health management.
Responsibilities
- Conduct comprehensive patient assessments to determine healthcare needs.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor vital signs and manage medical equipment as necessary.
- Provide education to patients and families regarding disease processes, medications, and health maintenance.
- Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services.
- Document all patient interactions accurately in compliance with regulatory standards.
- Assist in critical care transport when necessary, ensuring safe and effective patient transfers.
Experience
- Previous experience in hospital medicine or medical-surgical nursing is preferred.
- Knowledge of physiology and medical terminology is essential for effective communication and patient care.
- Experience in critical care settings such as PICU or critical care transport is advantageous.
- Familiarity with medical office procedures can enhance the efficiency of patient management.
- Strong clinical skills in ventilating patients and understanding inhalation therapies are beneficial.
