Position Summary: The primary focus of the Benefits and Wraparound Specialist is to assist and support Big Valley tribal members who are experiencing homelessness and/or at-risk for homelessness, including individuals who may have complex health or behavioral health conditions, with a variety of services to address homelessness within the Big Valley Rancheria and support the housing programs of the Big Valley Rancheria (BVR) Housing Department. Overall duties and responsibilities involve working directly with housing program participants, including advocating benefits available to homeless tribal members, assisting with immediate rehousing needs, and other supportive housing services.
Qualifications
Essential Duties and Responsibilities:
- Conduct initial interviews, comprehensive assessments, outreach efforts and utilize intake forms to determine appropriate housing options and other supportive services.
- Evaluate participants for eligibility in various housing programs, including the Housing & Disability Advocacy Program, including assisting participants with applying for benefits in governmental programs such as Supplemental Security Income (SSI), or Social Security Disability Insurance (SSDI), or any other available and appropriate programs.
- Assist participants in requesting medical evidence, if needed, and prepare letters and/or forms related to disability benefit applications as needed.
- Assists program participants to get "document ready" by supporting them to obtain IDs, social security cards, birth certificates, and other key documents needed to successfully obtain housing and other appropriate supportive services, including letters of reference.
- Develop and maintain good working relationships with a variety of healthcare providers and government and social service agencies, and community-based organizations to support tribal members in receiving needed services.
- Works with program participants and the Services & Care Coordinator to develop a housing service plan and/or housing location plan.
- Work with potential landlords to assist rehousing program participants.
- Coordinates the emergency needs of program participants within the BVR Housing Department program services available.
- Coordinates housing search activities and attends housing unit/apartment viewings with program participants, assists with overall housing stability.
- Work with program participants to develop a housing stability plan and budget.
- Track assessment and data collection into a management information system for reporting.
- Provides program participants with support when housing-related issues are identified.
- Conducts on-going assessments to identify needs or barriers to supportive housing.
- Performs home visits, eviction prevention, budgeting, life skills coaching, and other housing retention services to support participants with sustaining permanent housing.
15. Prepares, maintains, and updates participant's case files related to the case for review and recordkeeping and/or reporting purposes. Ensures timely completion of case notes.
16. Always maintain program and client confidentiality and integrity.
17. Any other duties as assigned by the Housing Director.
