What are the responsibilities and job description for the Nurse Navigator - REI - Bozeman position at billingsclinic?
The REI/IVF Navigator contributes to the Physician’s effectiveness by identifying patient care issues; provides information and commentary pertinent to the patient care plan; recommends possible options as appropriate and implements physician directives. Establishes an empathic patient experience by providing emotional and psychological support to all our patients. This position will assist with in-office procedures and do basic hands-on patient care as needed.
Essential Job Functions
- Serves as a lead in the direction of REI RN services and is a steady influencer and a source of sound judgment.
- Conducts the education session for patients scheduled for identified REI clinics to ensure that all patients receive appropriate education regarding their clinic visit. Works with ancillary services to ensure a comprehensive patient education and preparation is achieved and maintained on an ongoing basis.
- Responsible for creating patient education and process in alignment with the multidisciplinary team.
- Coordinates the episode of care for all REI patients including; scheduling the patient, obtaining previous pertinent medical records, coordinating pre-visit informational packets, coordination with physician offices to ensure timeliness of diagnostic services and pre-visit workups are complete. Works with multidisciplinary team and patient to assure prior authorizations are obtained.
- Follows all patients and coordinate patient care during the REI clinic visit to facilitate a seamless delivery of care process.
- Follows up to ensure patient’s needs have been met.
- Functions as a member of an integrated professional team in the ongoing accurate assessment of patient's needs through meeting with physicians, advanced practice providers, genetic counselors and nurse caregivers to promote the development, implementation and evaluation of the care plan on an ongoing basis.
- Collaborates with physicians, nursing, genetic counselors, marketing department and all members of the healthcare team through team conferences and one-on-one meetings to ensure a quality care-delivery process for patients across the continuum.
- Responsible for development, revision and implementation for all patient education materials and notebooks to educate patient and families on their care incorporating an interdisciplinary approach and utilizing best practices and research findings. Ensures all materials are standardized and consistent with standard order sets and clinical pathways.
- Accountable for working with physicians and staff to update on an annual basis or as needed to maintain high quality of care standards.
- Participates in staff education on an individual and or group basis as needed.
- Works with the team to identify and track key performance measures including but not limited to: patient satisfaction, physician satisfaction, treatments and specific outcome tools as necessary. Works with Quality Management to distribute reports to key stakeholders and participate in process improvement initiatives as required.
- Serves as a resource to all members of the health care team related to provision of care, enhancing assessment skills of others, establishment of appropriate care standards, and determination of realistic goals for optimal patient outcome.
- Maintains a current knowledge of changes in women’s health patient care management standards and takes appropriate measures to incorporate into performance systems.
- Promotes evidence-based practice in self and others by reading, interpreting and evaluating research for utilization in practice.
