We are looking to hire one indiviual from each county (NCC, Kent, & Sussex).
Are you passionate about supporting individuals affected by substance and opioid use across our state? We’re seeking a dedicated and experienced Substance Abuse Navigator to join our team and take charge of guiding both clients and case management staff. In this key leadership role, you’ll oversee case management operations, mentor team members, and ensure high-quality care and support for clients navigating recovery and treatment challenges. You’ll operate at the intersection of healthcare, social services, and advocacy, driving impactful results for individuals and families in need.
Key Responsibilities
- Mentorship: Provide mentorship to your assigned case management team, assist with onboarding and training to ensure consistent delivery of quality care.
- Client Assessment Oversight: Ensure thorough evaluation of clients' health, social, and emotional needs to identify areas requiring support, while mentoring staff on best practices for assessments.
- Care Plan Development & Review: Collaborate with case managers to develop effective, recovery-focused care plans tailored to each client’s unique needs and goals.
- Service Coordination Management: Oversee coordination efforts to connect clients with treatment programs, counseling services, housing support, financial assistance, and other necessary resources.
- Monitoring & Evaluation: Regularly review care plans and client progress with case managers, making necessary adjustments to ensure optimal recovery outcomes.
- Client Advocacy: Advocate on behalf of clients to ensure access to high-quality care and support, while building strong relationships with service providers and community organizations.
- Compliance & Documentation: Maintain oversight of accurate and confidential documentation of client interactions, care plans, and progress, ensuring compliance with laws and regulations.
- Collaboration & Community Engagement: Foster relationships with healthcare providers, addiction specialists, social workers, and community programs to strengthen the network of support services.
- Client & Staff Education: Provide education and guidance to both clients and team members on treatment options, recovery systems, and evidence-based strategies for successful outcomes.
Qualifications
- Minimum 1-3 years of experience in case management, substance use treatment, or related social services.
- Proven leadership experience in managing and mentoring teams.
- Strong knowledge of addiction recovery, treatment systems, and available resources.
- Excellent communication and interpersonal skills to effectively engage with clients, teams, and stakeholders.
- Ability to design, implement, and evaluate care plans while managing team caseloads.
- Proficiency in maintaining accurate and organized client and operational records.
- Commitment to providing compassionate, client-centered care in a recovery-focused environment.
- Understanding of laws and regulations related to substance use treatment and social services.
- A valid driver’s license and reliable transportation may be required for some positions.

- Evenings as needed
