The Spiritual Care Lead is responsible for providing leadership and guidance for the hospice spiritual care team consistent with the mission vision, and values of Bloom Hospice’ quality outcomes and regulatory standards. The Spiritual Care Lead provides spiritual, emotional, and pastoral support to patients in hospice care and their families, regardless of religious affiliation or spiritual beliefs. They work as part of an interdisciplinary hospice team to address the holistic needs of individuals nearing the end of life.

Hospice Spiritual Care Lead Responsibilities:
- Conducts spiritual assessments to understand patients' and families' beliefs, values, and needs.
- Offers spiritual guidance and emotional support aligned with the patient’s faith or worldview.
- Provides rituals or sacraments as requested, including prayers, blessings, or other spiritual practices.
- Assist families in coping with grief, loss, and the emotional challenges of caregiving.
- Facilitates family discussions regarding end-of-life issues, legacy planning, or unresolved conflicts.
- Provides anticipatory grief support for patients and family.
- Works as part of the hospice care team, including doctors, nurses, social workers, and volunteers, to create a holistic care plan.
- Offers insight into spiritual or cultural needs during team meetings.
- Advocates for patients’ religious or spiritual preferences.
- Maintains accurate records of spiritual care visits and interventions in compliance with hospice policies.
- Contributes to care plans by documenting spiritual care goals and outcomes.
- Collaborates with local clergy or spiritual leaders when specialized care is requested.
- Educates community and staff about the role of spiritual care in hospice.
- Assists with memorial services or support group facilitation as needed.
- Adheres to Bloom standards and continuously improves processes and services.
- Manages a modified caseload, providing spiritual needs and consultation to patients, families, and caregivers in a timely and supportive manner.
- Assists in the development, revision, and implementation of policies and procedures to guide the work of the SC team.
- Supports SC team with daily tasks including, but not limited to, caseload management, scheduling, EMR assistance, documentation, procedures, delegation of workload, resources, and case review.
- Identifies staff development and training needs, mentor’s SC team.
- Participates in the interviewing and onboarding process within the framework of SC preceptor.
- Participates in staff meetings, training sessions, and continuing education to stay updated on best practices in hospice care.
- Performs related work as assigned.
Hospice Spiritual Care Lead Minimum Qualifications:
- Master’s degree in divinity, theology, pastoral care, or a related field
- Ordination or endorsement from a recognized faith tradition.
- Previous hospice experience.
- Excellent communication and interpersonal skills.
- Ability to effectively collaborate with interdisciplinary team in providing patient care.
- Solution driven, creative and resourceful problem-solving skills.
- Ability to function independently and effectively anticipate needs and problems.
- Skilled in organizing and prioritizing work.
- Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
- Must possess professional image, positive attitude, enthusiasm and self-motivation.
- Comfortable in maintaining professional boundaries.
- Proficient computer skills.
- Awareness and sensitivity to cultural diversity.
- Knowledge of self-care, personal boundaries; abilities, limits and inner resources.
- Current CO Driver's license and automobile insurance.
- Clinical Pastoral Education (CPE) certification or equivalent training.
- Bilingual abilities a plus.
