Population Health Case Manager
As a Case Manager with BCBSWY, you serve as an essential member of the Population Health team supporting members in their healthcare journeys. This role requires the ability to support a member-centric approach through collaboration with internal and external partners. This role also requires the ability to build rapport with healthcare professionals and foster strategic relationships within our Wyoming communities.
As a member of the Population Health Team and an employee of BCBSWY, you are an integral member of our operational organization. You will be responsible for providing clinical assessments to members leading to care planning, implementation of gaps in care programs, coordinating with internal and external resources, and monitoring the progression of members within the program.
As a recognized BCBSWY employee, we count on you to positively serve in the role of “Brand Ambassador” within the local community and/or geographical region. Within this role, you will be tasked with presenting clinical programs to key stakeholders.
To be successful in this role, you will need knowledge of managed care principles. You must possess contemporary knowledge of the provider community and their functional environment as well as the overall health care landscape. Competency with data analysis, interpretation, and application will play a crucial role in the day-to-day activities assigned.
Minimum qualifications include a bachelor’s degree in nursing or master’s degree in social work, at least three (3) years prior experience in case management or five (5) years in a directly related healthcare field working in medical and clinal services, insurance, or health insurance exchange, and an active driver’s license. Preferred qualifications include a Certified Case Management (CCM), a combination of managed care and provider-based experience, and a background in maternity or behavioral health.
There may be evening and weekend hours on a periodic basis and moderate in-state travel, with some overnight travel possible.

Our positions are all based in Wyoming. Depending on the department and the position, eligible employees may be offered limited In-Office/WFH flexibility (for those positions that are offered limited WFH, there will be a required number of In-Office days per week/month depending on department). Executive level employees are required to reside full-time in Wyoming.
