Blue Ridge Health is seeking an ambitious and organized Substance Use Disorder Specialist to join our integrated care team. This full-time position is essential in supporting patients on their recovery journey by providing person-centered case management services. The SUD Specialist will play a key role in both the Opioid-Based Outpatient Treatment (OBOT) Program and our broader Medication for Opioid Use Disorder (MOUD) services, ensuring seamless care coordination and comprehensive support for individuals managing substance use disorders in Rutherford and Polk Counties. Responsibilities include:
- Serves as the primary point of contact for referrals, appointment scheduling, and linkage to appropriate levels of care.
- Screens and enrolls patients in the Opioid-Based Outpatient Treatment (OBOT) Program.
- Leads the client intake process, including screening, education/review of program expectations, and completion of enrollment paperwork.
- Coordinates care for clients throughout their time in the program.
- Provides complex care coordination across multiple life domains and disciplines, including communication with community partners, referrals to outside agencies, and management of other care needs.
- Coordinates care with our regular Medication for Opioid Use Disorder (MOUD) services to ensure seamless patient support.
- Uses motivational interviewing and shared decision-making to address barriers and promote full participation in treatment.
- Attends warm handoffs when patients in the clinic request contact with the OBOT program.
- Acts as the primary source for supporting the OBOT team in patient tracking.
- Ensures effective collaboration and communication between members of the healthcare team to support care coordination activities and program goals.
- Consults with medical and behavioral health providers and other team members to resolve client care issues.
- Collects, organizes, and provides the OBOT team with data necessary to support program development, grant reporting, and quality measurement efforts.
- High School Diploma or equivalent required.
- Certified Alcohol Drug Counselor (CADC) or similar credential/experience in substance use treatment required.
- Education in the Medical/Human Services field preferred.
- Excellent interpersonal and communication skills, both oral and written.
- Ability to work effectively as part of a multidisciplinary team.
