Designated Coordinator
Salary $42,640- $44,200 DOE
At Blue Sky Inc., we are seeking a dedicated full-time Designated Program Coordinator to join our team at our Brainerd Day Services location.
As a Designated Coordinator, you play a vital role in enhancing the lives of the individuals we serve, providing quality support that empowers their skill development, health, and personal dignity.
Essential Qualifications:
- All applicants must be able to pass a background check under Minnesota 245D (Criminal Background Studies of individuals affiliated with Programs Licensed by the Department of Human Services)
- A valid driver’s license with a clean record
- This is a leadership role; this person can effectively use a computer and software, have good written and oral communication skills and a strong knowledge of the Direct Support Professional staff position.
- Ability to respond quickly to phone calls, texts, or emails regarding requests and expectations from supervisors.
- Reliable transportation that is insured in the state of Minnesota.
Education / Experience Required:
- A baccalaureate degree in a field related to human services and one year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older.
- An associate degree in a field related to human services and two years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older.
- A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older.
OR
- A minimum of 50 hours of education and training related to human services and disabilities.
AND
- Four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Responsibilities:
- Lead and attend support team progress meetings to ensure coordinated services.
- Collaborate with the extended Support Team and Directors for effective service coordination.
- Design and implement support programs aligned with individual and family goals.
- Ensure adherence to Service and Support Plans through staff training and assessment.
- Oversee and provide ongoing training to Direct Support Professionals (DSPs) and guide their growth.
- Maintain accurate documentation in a timely manner.
- Other duties as outlined in the full job description.
