This position is responsible for providing Community Living Supports for children enrolled in the YES Waiver. This position will work in full partnership with team members to help the youth meet goals identified in the YES Waiver family team meetings. Community Living Support Provider is a mental health professional / skills trainer that facilitates the modelling, coaching and social skill building in the individual’s community with children 3-18 in Youth Empowerment Services. CLS services are used to increase or maintain personal self-sufficiency, helping an individual to reach his / her goals of community inclusion and participation, independence, or productivity. CLS services may be provided in the individual’s residence or in community settings (including, but not limited to, libraries, city pools, camps, etc.)

Community Living Support Specialist is responsible for care that is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.

The person in this position requires an aptitude for working with people who have diagnoses of bipolar disorder, major depressive disorder as well as schizophrenia, schizoaffective disorder and other psychotic disorders; this position also requires knowledge of psychotherapeutic medications as well as evidence-based practices and contraindications in the promotion of recovery for individuals with severe and persistent mental illnesses. This position requires knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.

Minimum Qualifications : A Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field. Knowledge of mental illness as defined in the DSM-IV is required. Must have a valid Texas driver's license.

Preferred Qualifications : One year of experience in mental health assessment and / or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.
