Medix is now hiring a Community Supports Manager based in the Monterey County area!
The Community Supports Manager will conduct weekly caseload audits for program clientele and ensure appropriate and ethical documentation is maintained for all cases within the electronic health record system. They will oversee, evaluate and / or revise the care plans as needed for clients enrolled in ECM services. The Community Supports Manageer will ensure compliance with state and federal laws and regulations, in addition to the Policies & Procedures set-forth by Champions, DHCS, and managed care plans.
RESPONSIBILITIES AND DUTIES
- Oversee resource allocation, record-keeping, and compliance with program goals and regulations.
- Evaluate and enhance program operations to improve effectiveness and meet client needs.
- Coordinate collaborative activities and projects to optimize resources and client responsiveness.
- Supervise ECM and Housing staff, identify training needs, and deliver relevant training.
- Develop and manage systems to maximize ECM and Housing Program effectiveness.
- Collaborate with partners to achieve program goals and comply with guidelines.
- Participate in budget preparation, resource planning, and expenditure monitoring.
- Perform screenings, create individualized care plans, and coordinate case management.
- Attend meetings to address client needs and integrate services.
- Assist with grants to expand CalAIM services geographically and in scope.
- Participate in relevant meetings, trainings, and conferences.
- Ensure cultural sensitivity and maintain client confidentiality in all interactions.
- Perform additional duties as assigned.
