What are the responsibilities and job description for the Crisis Clinician – Williamson County Homeless Prevention position at Bluebonnet Trails Community Services?
Williamson County Homeless Prevention (WCHP) provides crisis intervention and assessment, rehabilitation specialist, and care coordination services to a specialty population of individuals who are chronically homeless.
The program partners with permanent supported housing programs such as Pecan Gardens and Community First! Village. WCHP’s primary referral sources are law enforcement, the county jail, and behavioral health hospitals located in Williamson County. The core components of a crisis continuum are somewhere to call, someone to respond, and a place to go. The Bluebonnet Trails WCHP team has a key role in providing quick, in person response to those in need, and identifying options for next steps.
Our Vision is Healthy and Fulfilled Lives, and WCHP offers a team of dedicated clinicians supporting chronically homeless adults and youth in an assessment of eligibility for interim and permanent housing options, mental health, substance use, and medical care. The WCHP also provides case management support and assistance with the completion of obtaining vital documents, benefits completion, and employment services.
Our Mission is to continuously improve the health and independence of the persons we serve, and the WCHP provides timely access to care for basic needs, as well as solution-focused options that reduce utilization of local law enforcement, jail and hospitalization for the people enrolled in the program.
Critical Impact:
The WCHP team provides connection to interim and permanent supported housing as well as on-site support in permanent supported housing locations. The Crisis Clinician is the first point of contact in delivering care to residents of Pecan Gardens, collaborating with staff on site at Pecan Gardens, and communicating across interdisciplinary teams. The Crisis Clinician serves as the case manager to all open clients at Pecan Gardens and as the care coordinator and crisis responder for all residents at Pecan Gardens. Additionally, the Crisis Clinician provides support to WCHP clients in the community, interim housing, or in permanent housing at Community First! Village.
The crisis clinician provides person-centered care, including crisis intervention, routine and intensive case management, supported housing and employment, care coordination, and assistance with benefits completion; and participates in 360 degree feedback with direct supervisor, including collaboration on individualized support, goal setting, and coaching; builds relationship with colleagues, learning about options and resources at BTCS and in the community; ensures responsiveness to needs to a changing and growing program, maintaining flexibility as the program develops and continues to improve. As a Crisis Clinician, rotates once a month into the Mobile Crisis Outreach Team’s after hours on call in the Northern Hub
Location: Williamson and Travis Counties
Salary: $25.23 – $37.48/hr
Applicable Differentials:
Up to $3.50/hour – Previous crisis experience
Up to $2.00/hour – previous experience in a similar role with a community center
Up to $4.00/hour – Tenure with BTCS
.75/hour – Bilingual
$1.00/hour – Masters Degree in Human Services
$1.00/hour – LPHA licensures
$200 Monthly Service Incentive
