PACE Outreach Coordinator
Location: Lexington, KY (Serves Anderson, Fayette, Franklin, Jessamine, and Woodford counties)
Schedule: Monday-Friday 8am-4:30pm, with occasional evening and weekend community events
Note: This is a hybrid remote position and may be required to work in office upon hire during the introductory period.
As a BCN PACE “Program of All-Inclusive Care for the Elderly” team member, you will be a part of a program providing comprehensive health care services for Geriatric participants who can still live safely at the place they call home. Team members who work within our PACE Center can get to know our participants long-term and impact their quality of life. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for our participants and connect them with all aspects of medical care, social care, and support services, including everyday tasks to PACE participants at home or at the PACE Center. We Offer
The Outreach Coordinator is responsible for the development, implementation, and monitoring of sales and outreach strategies to increase referrals of eligible PACE participants to Bluegrass PACE Care. The focus of this role is assuring access to PACE and other BCN services through successfully developing and executing a targeted sales plan that focuses on cultivating referrals through education and relationship building with senior housing property managers, providers, health care agencies, and community members within the service area. This role works closely with the PACE Participant Enrollment Specialist to facilitate the intake process to ensure a smooth hand-off between the referral source and the PACE team. Works proactively with other clinical service line leaders and staff to identify patients within BCN who are potentially eligible for PACE services and assists in the smooth transition of those who are eligible from existing BCN service lines to the PACE program. Works cooperatively and collaboratively with Executive Director of PACE Operations and Director of Provider Strategy and Community Engagement to ensure alignment on outreach strategy initiatives and outcomes.

Requirements
- Bachelor’s degree or above, preferably in a healthcare or human services field and a minimum of two years’ experience in direct sales, healthcare experience preferred; OR a combination of education and/or experience.
- Must have at least one year of experience working with the frail and elderly.
- Food handling permit required, to be completed upon hire.
- Excellent communication skills, including face-to-face, telephone, and computer skills.
- Strength in attention to detail and organization.
- Must be able to interact successfully with participants, families, and other agencies to present a positive and professional image.
- Ability to exercise judgment and discretion in confidential matters.
- Excellent public speaking skills.
- Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
- Ability to work unsupervised exercising a high degree of discretion & independent judgment.
- Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.)
- Ability to travel within community.
- Available to cover evening and weekend events as needed.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
