Job Summary:
We are seeking a compassionate and dedicated Personal Care Assistant to join our team. The successful candidate will be responsible for providing high-quality patient care in a home health setting. The ideal candidate will have experience in caregiving, patient care, and meal preparation, with a strong focus on creating personalized care plans that cater to our patient's unique needs. The Personal Care Assistant will work closely with the patient who is a quadriplegic and may be required to lift and move patient as needed. This role is ideal for individuals who are passionate about improving the lives of others and have a strong commitment to delivering exceptional patient care.
Responsibilities:
- Develop and implement personalized care plans for the patient.
- Provide patient care and support in a home health setting
- Assist with meal preparation and ensure patient receives a nutritious diet
- Monitor patient condition and report any changes or concerns
- Assist patients with daily living activities, including bathing, dressing, and grooming
- Lift and move patient as needed to ensure safe and efficient care. Equipment for moving the patient is available on site.
- Work closely with patient who have complex health needs and require specialized care
- Provide emotional support and companionship to patient and their family
- Maintain a clean and safe living environment for patient
- Collaborate with healthcare professionals to ensure high-quality patient care, when needed
Qualifications:
- High school diploma or equivalent required
- Experience in caregiving or patient care
- Knowledge of patient observation and care plans
- Ability to lift and move patients as needed
- Experience working with patients who have complex health needs
- Strong communication and interpersonal skills
- Ability to work in a home health setting and maintain a clean and safe environment
- Certification in patient care or caregiving preferred
- Experience in meal preparation and nutrition
- The Background will require specific documentation (Drivers License, Social Security Card and HS Diploma/GED)
