- Medical management for acute / episodic, rehabilitation patients, as well as long term chronically ill patients. Weekend call availability is required.
- The practitioner will provide ongoing medical management and acute interventions for patients in the sub- acute rehabilitation facility, long term care and memory care areas.
- Position requires work as a member of the interdisciplinary team, developing updates and evaluating medical treatment plans, medications, and other necessary care.

Minimum Qualifications :
Education : Nurse Practitioner (NP) Masters level program accredited by an appropriate agency recognized by the South Carolina State Board of Medical Examiners (SCSBME).
Experience : 1-2 years of experience as an APP; Geriatric, Acute Rehabilitation or Sub-Acute experience preferred.
