Borinquen Medical Centers is based in Miami, FL and is seeking to hire a full-time Program Manager to join our team! The Program Manager is responsible for managing the day-to-day operation of the CDC-funded HIV testing program. The position is also responsible for training and supervising the project staff. The management of this project includes determining outreach and recruitment activities, ensuring the fidelity of program activities and service delivery, monitoring program incentives, and collecting data and writing project reports. In addition, this position will coordinate and manage the community engagement group/community advisory board (CEG/CAB) meetings and activities. This individual will also be in charge of providing medication adherence counseling and guidance in conjunction with the Health/peer navigator to ensure adherence to meds and challenges for patients in HIV care and for those in PrEP. The individual is responsible for developing a multi-step process which ensures coordination of the program meeting the goals set for the team while ensuring medication adherence and medication reconciliation and coordination of care with those patients diagnosed and treated in care, with the PCT and the clinical pharmacist at Borinquen.
Tasks and Responsibilities
- Manage the day-to-day operations of the program, coordinating the implementation of all pertinent trainings including but not limited to PROMISE and DOH 500-501.
- Track ( ) HIV patients and provide at least quarterly follow-up telehealth or face to face visit regarding medication adherence.
- Implements goals and tracks program goals as outcomes
- Enter monthly data performed by the team and ensure program evaluation and workplan deadlines are met.
- Provides to Quality Manager and CQO weekly calendar of activities and weekly outcomes.
- Supports and coordinates cluster response and targeted community detection efforts.
- Coordinates the overall interdisciplinary plan of care for a patient, ensuring the team performs testing, risk reduction and linkage to care, while ensuring medication adherence for HIV and PrEP.
- Ensures staff is cross trained and schedules meet goals of the program.
- Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
- Enrolls HIV positive clients in early intervention services through the primary care services.
- Assists the client in goal planning, identifying short- and long-term goals, task and time frame as they relate to service needs.
- Follows all linked to care patients, ensuring Health/Peer Navigator coordinates their care continuum. Ensures follow up calls are performed by the outreach team @ 15 days, 30 days and at 45 days to ensure follow up visit is performed.
- Coordinate with Clinical Manager’s: Reschedule patients who have not kept important appointments with the Patient Care Team.
- Send appropriate follow–up letters to patients who missed appointments
- Ensures that medication adherence includes ART, PrEP and medications to manage associated co-morbidities
- Tracks patient’s and data entered as required by the program and evaluating outcomes including viral load suppression, depression screenings and annual wellness visits.
- Monitors and follows up on service delivery.
- Monitor client adherence to the care plan and medication regimens.
- Coordinates and participates interagency activities with the Health Navigator for client specific services.
- Monitor’s quality of services, resolving problems regarding access to service and assisting in the development of alternative arrangements for filling unmet services needs.
- Provides clients with community referrals, providing name and location of available name and location of available services, nature and hours of services and application procedures.
- Conducts discharge planning and case closure with clients as appropriate, connecting client to other agencies for continuation of services as needed.
- Create and modify documents for activities and prepares reports as required by the program.
- Support staff in assigned project-based work.
- Other duties as assigned by immediate supervisor as required.
REQUIREMENTS
- Associate in science, biology, social work or healthcare administration. Bachelor’s degree would be best.
- Three years of experience in healthcare services including PLWHA.
- Strong interpersonal skills.
- Ability to interact effectively with consumers, administration, faculty, and staff.
- Handle the information with high level of confidentiality.
- Demonstrate AIDET.
- Compliant with BHCC Standards.
