What are the responsibilities and job description for the Clinical Social Worker, MSW Outpatient Cardiology position at Boston Children's Hospital?
This full time Outpatient Cardiology Clinical Social Worker provides social work support to patients from birth to adulthood and their families within cardiology clinic with a high incidence of medical and social complexity.
The Outpatient Cardiology Clinical Social Worker will be responsible for :
- Performing comprehensive psychosocial assessments, crisis intervention, short-term counseling, case management, liaising with outside agencies such as DCF, and complex resource coordination with referral to community agencies.
- Collaborating and consulting with the multidisciplinary team around management of psychosocial issues including coping with diagnosis, impact of illness on family members, parenting and caregiver concerns, grief, loss and / or end of life issues, trauma, child protection concerns and domestic violence.
- Assisting families who live locally, nationally, and internationally who are traveling to the Heart Center to access highly specialized and innovative treatment for the care of complex cardiac conditions.
- Working with patients transitioning to adulthood and assessing needs around guardianship, insurance concerns, benefits, and mental health supports.
- Program development, teaching, advocacy, and participation in initiatives aimed at improving patient outcomes.
