Summary

We are seeking a Social Services Manager to join our team at Boulevard Rehabilitation Center. This role is pivotal in overseeing social services programs, managing staff, and ensuring that residents receive comprehensive support tailored to their individual needs. Located in Boynton Beach, FL, this position contributes significantly to our commitment to high-quality care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

-Recruits, hires, provides orientation/training, and retains a sufficient number of qualified staff to carry out the responsibilities of the Social Services department. Ensures employee performance meets or exceeds expectations and periodic performance appraisals are conducted on a timely basis.

-Collects and assesses data relevant to patients’ psychosocial needs, risk factors for psychosocial deterioration and responses to interventions.  Implements social service interventions that achieve treatment goals, address resident needs, link social supports, physical care, and physical environment to enhance quality of life.

-Completes required forms and documents in accordance with company policy and state and/or federal regulations.  Coordinates discharge planning.

-Facilitates advanced directive decision-making process. Coordinates Family Council.

- Coordinates grievance complaint process. If appointed Grievance Official, will be responsible for overseeing the grievance process.

- Provides duties on a full-time basis in centers with more than 140 beds.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Current state licensure as a Social Worker is preferred.
• Proven experience in program management within a healthcare or rehabilitation setting.
• Strong supervisory skills with the ability to lead a team effectively.
• Excellent relationship management skills for working with diverse populations.
• Familiarity with budgeting processes and administrative tasks is beneficial.
• Experience in behavioral health or working with individuals with developmental disabilities is a plus.

Pay: $49,996.00 - $54,378.00 per year
