What are the responsibilities and job description for the Social Services Specialist position at Bowen Health?
The Social Services Specialist provides pertinent care to ensure the bio-psycho-social-spiritual needs of the patients are met during their hospital stay. This position works closely with patients and their caregivers to identify needs and concerns contributing to the hospitalization and assist in planning for stability in the community following their discharge. This role is a critical link for treatment and discharge planning for the interdisciplinary treatment team.
Duties/Responsibilities:
- Facilitate daily Interdisciplinary Team Meetings.
- Complete holistic bio-psycho-social-spiritual assessments of patient needs.
- Create and continually update the treatment plan during the patient’s stay.
- Meet with patients one-on-one to utilize evidence-based practices to address treatment goals.
- Obtain collateral from family or other support persons to support safe discharge planning.
- Facilitate meetings with patient’s family, caregivers, or outpatient treatment teams.
- Provide discharge planning to patients to ensure continued access to care and support.
- Facilitate therapeutic group activity sessions.

- Support the interdisciplinary treatment team and facilitate open communication about patient care.
- Complete all documentation on the day of service.
- Ability to manage multiple patient needs in a short period of time and manage time effectively.
- Comfort working with individuals with serious mental illness and supporting the treatment team during a behavioral health emergency.
- Strong organizational and communication skills.
- Present self with professionalism at all times.
Required Qualifications:
- Education: Bachelor’s Degree required, preferably in Social Work; Master’s Degree preferred
- Certificate or Professional License: No certificate or professional license required.
- Experience: Prefer three (3) years in a mental health setting.
- Valid driver’s license and active liability insurance.
- Satisfactory results from criminal and driving record background checks.
