This job description outlines the role and responsibilities of a Pharmacist working in a healthcare facility, reporting directly to the Pharmacy Manager. Here’s a breakdown of the key aspects of the position:

General Summary of Responsibilities:

The pharmacist is responsible for ensuring the safe, effective, and appropriate use of medications for patient care. This includes dispensing prescribed medications, providing drug information, collaborating with healthcare teams, and promoting quality patient care practices.


Essential Job Functions:

  1. Collaborating with a Healthcare Team:
    The pharmacist works closely with physicians, nurses, and other healthcare professionals to ensure the best pharmaceutical care for patients.

  2. Drug Therapy Review:
    Regularly assesses drug therapy to ensure it is appropriate based on the patient’s condition, including reviewing dosing, medication regimen, possible interactions, and drug duplications.

  3. Medication Order Clarification:
    Communicates with prescribers about any unclear medication orders, adjusts dosages, and documents changes accordingly.

  4. Care Conferences & Rounds:
    Actively participates in interdisciplinary care conferences and patient rounds, especially in areas like swingbed care (patients in recovery or rehabilitation).

  5. Medication Reconciliation:
    Assists in ensuring patients' medication lists are accurate, particularly during hospital admissions, transfers, and discharges.

  6. Formulary Management:
    Promotes the use of the hospital's formulary to ensure cost-effective and safe medication use, while also managing non-formulary requests when necessary.

  7. Patient Education:
    Educates patients on their prescribed medications, their purposes, and any potential side effects.

  8. Emergency Response:
    Responds to medical emergencies (like Code Blue), preparing medications as necessary.

  9. Nursing Staff Education:
    Provides training and educational materials on medication-related issues for nursing staff.

  10. Committee Participation:
    Contributes to committees such as Medicine, Infection Control, and the Pharmacy and Therapeutics (P&T) Committee to help guide hospital policies and practices.

  11. Antimicrobial Stewardship:
    Helps analyze and promote antimicrobial stewardship practices, ensuring the responsible use of antibiotics and other medications.

  12. Community Outreach:
    Engages in health outreach efforts, including drug take-back programs and health fairs.

  13. Compounded Sterile Products:
    Tracks and assists in the preparation of sterile medications for patient care.

  14. Medication Verification:
    Double-checks drugs before they are loaded into medication dispensing systems like Omnicell.

  15. Billing Assistance:
    Assists with medication billing through the hospital's electronic health records system.

  16. Drug Shortage Management:
    Identifies and researches strategies for managing drug shortages, ensuring continued patient care.

  17. Adverse Drug Reactions (ADR) Reporting:
    Monitors, reports, and documents any ADRs or medication errors, working to mitigate risks.

  18. Equipment Management:
    Assists with maintaining databases for equipment like IV pumps and medication dispensing cabinets.

  19. Pharmacy Manager Support:
    Supports the pharmacy manager in executing operational responsibilities and maintaining compliance with regulations.

  20. Quality Improvement:
    Actively participates in initiatives to enhance hospital performance and patient safety.

  21. Policy Development:
    Contributes to developing policies to ensure compliance with accreditation standards like those from The Joint Commission and CMS.

  22. Other Duties:
    Performs additional duties as assigned, contributing to the overall mission of the healthcare facility.


Job Qualifications:

Education:

  • Required: Bachelor of Science in Pharmacy.
  • Preferred: Doctor of Pharmacy (PharmD) degree.

Experience:

  • Required: No prior experience necessary.
  • Preferred: At least one year of experience in a hospital setting.

Licenses/Certifications:

  • Required: A valid Nebraska Pharmacist license or the ability to become licensed in Nebraska within 90 days of hire.

Personal Skills:

  • Strong verbal, written, and interpersonal communication skills.
  • Demonstrated initiative and follow-through in tasks.
  • Ability to contribute positively to a team-oriented environment.
  • Analytical thinking to assess data, make judgments, and provide recommendations.
