Job duties :

Transcribe and take appropriate action on physician’s orders as directed including but not limited to Care Plan changes.

Assist with performing all admitting and discharge assessments, and complete all other necessary paperwork.

Assist / direct visitors.

Clean equipment.

Help monitor all aspects of patient care.

Initiate requests for consultation or referral. Respond to requests from the resident, physician, or nursing staff.

Cooperate with other resident services when coordinating nursing services to ensure that the resident’s total regimen of care is maintained

Coordinate and prepare MDS assessments for all residents.

Oversee the transmission of MDS resident assessments.

Prepare, monitor, and update assessments, Care Area Assessments, and Care Plans, as necessary.

Participate in Care Plan meetings with residents and family members.

Be aware of residents’ exit to and reentry from hospitals

Minimum Education :

MDS Coordinator will have completed all educational requirements as required for licensure and / or certification.

Minimum Work Experience :

The facility prefers that all employees have prior health care experience and be knowledgeable of the essential job functions they will be performing.

It is preferred that employees have a minimum of six months of long-term care experience.

Certification :

It is preferred that the MDS Coordinator be certified in MDS 3.0.
