GENERAL DESCRIPTION :

Performs nursing practices within the scope of the Nurse Practice Act of the State of Ohio. Performs daily duties in accordance with job standards and as directed by the Health Services Supervisor. Follows prescribed procedures and policies of the facility and Federal, State, and local authorities, to ensure that quality nursing services are provided at all times.

MAJOR DUTIES AND RESPONSIBILITIES :

  • Protects and maintains confidentiality of residents, information and resident's rights.
  • Respects and shows kindness to all residents.
  • Maintains the standards of nursing care and implements the policies and procedures of the facilities nursing department including skin grids and treatments, preventative skin plans, preventative fall plans, incident reports and admission paperwork.
  • Maintains current knowledge and skills with respect to facility activities, nursing procedures, and technology.
  • Attends team care conferences as needed.
  • Provides oral instructions to nursing assistants.
  • Records all pertinent information in resident's records.
  • Assesses residents.
  • Provides resident's care and activities in accordance with their prescribed medical and nursing treatment plans.
  • Document administration of medications according to facility policies and procedures.
  • Functions as team leader and supervisor.
  • Supplies and interprets information, supports decisions, and facilitates communications with residents and their families, and nursing assistants and support staff.
  • Assures that resident's call lights are answered.
  • Receives and records oral telephone medication and treatment orders from physicians / physician's offices.
  • Reports changes in resident's health status to physician, resident, and / or responsible party.
  • Protects and maintains all work areas in accordance with facility safety policies.
  • Assures that floors are free of hazardous debris.
  • Performs daily tasks in accordance with infection control and standard precaution policies and procedures.
  • Follows policies and procedures related to body mechanics and lift systems.
  • Dresses in accordance with facility policy.
  • Assures residents individual care plans are being followed by reviewing nursing assistant assignment sheets.
  • Assures nursing assistant are completing their required documentation before shift ending including service check off lists, Restorative Care and Intake and Output records.
  • Assures rounds are being done with shift to shift change to prevent conflict.

NON-ESSENTIAL RESPONSIBILITIES :

  • Resupplies and maintains order of medication and treatment carts.
  • Occasionally changes schedule or works overtime.
  • Participates on one or more committees of the facility.
  • Occasionally attends activities in the community with a resident.
  • Occasionally transports blood and / or specimens to the hospital lab.
  • RESPONSIBILITY AND DECISION-MAKING AUTHORITY :

  • Decision making is limited to specific responsibilities as assigned by the Director of Health Services.
  • EDUCATION & EXPERIENCE :

  • Maintain current licensure from the State of Ohio
  • Attends in-services.
  • Maintain current CPR certification.

  • Lifts

  • Oxygen concentrators
  • Nebulizers
  • CPAP
  • Suction machine
  • REPORTING PREPARATION :

  • Oral and written report as prescribed in policies.
  • All changes in resident's condition.
  • All hazardous conditions in the facility, including equipment.
  • All accidents and or incidents observed.
  • SPECIFICATIONS - MENTAL / PHYSICAL :

  • Lifts routinely, objects weighing up to 50 lbs
  • Stands and walks on hard surfaces 6-7 hours daily
  • Bends, stoops, and turns often
  • Pushes and pull medication and treatment carts weighing up to 150 lbs. on a regular basis.
  • Pushes wheelchairs and recliners with residents on them, weighing up to 250 lbs.
  • Assists weight-bearing residents weighing up to 200 lbs. from bed to bedside commode, and to chair on a daily basis.
  • Exerts considerable force in pulling and pushing daily.
  • Hearing ability must be sharp to understand residents and their needs.
  • Visual ability must be sharp to understand residents and their needs.
  • Able to think clearly, and react calmly during emergency situations, and assist with possible evacuation of residents.
  • Work / tasks are performed mostly indoors.
  • During spring, summer, and fall, escort residents outdoors for on / off ground activities.
  • Outdoor activity areas are well-lighted and clean, with some exposure to dust, dirt, and organic substances.
  • Exposure to heat, air conditioning, and humidity as controlled by the facilities' central system, with variations in temperature of 4 degrees on either side of the normal 76 degrees.
  • There are constant exposures to blood, body tissues and fluids, with occasional exposure to hazardous materials and infectious diseases.
  • Adjusts to exposure of loud and unpleasant voices.
  • Exposure to constant stress of working with sick residents and their families.
  • Exposure to residents who are confused, irrational, agitated, and combative with abrupt mood swings, on a regular basis.
  • RELATIONS WITH OTHERS :

  • Maintains good relations with residents and their families.
  • Communicates clearly and maintains good relationship with staff.
  • LEARNING PERIOD REQUIRED :

  • Ninety (90) day probationary period.
  • 6-10 days of orientation. Determined according to the Nursing Staff Orientation Policy and Procedures.
