A highly reputable non-profit organization dedicated to providing superior healthcare to seniors is searching for an MDS Coordinator for its Manchester, New Hampshire location. An MDS Coordinator is a specially trained, licensed nurse who assesses residents using the MDS tool, which supplies valuable data on the patient’s cognitive ability, nutritional needs, behavioral patterns, and mood. This position is in a beautiful 51-bed skilled nursing facility that offers short- and long-term care in a warm and supportive environment.
Responsibilities:
- Oversee the MDS process, including assessment, triggers, and care planning.
- Verify the completion and accuracy of MDS before submission.
- Maintain current PPS/Medicare and Medicaid/Private assessment schedules for regular distribution to therapies, dietary, social services, activities, and nursing.
- Demonstrate current knowledge of nursing practices and procedures related to Long-Term Care, including state and federal regulations.
Qualifications:
- An ASN is required, and a BSN is preferred.
- Active RN license.
- 2 years of experience in long-term care.

- Monday-Friday, days
- $75,000-$85,000, depending on experience.
