Job Summary
The MDS Coordinator plays a vital role in ensuring the quality of care provided to patients by overseeing the assessment and documentation processes in accordance with regulatory guidelines. This position involves collaborating with interdisciplinary teams to develop comprehensive care plans and ensure compliance with all relevant standards. Duties
- Conduct comprehensive assessments of patients to determine their needs and develop individualized care plans.
- Ensure accurate completion of Minimum Data Set (MDS) assessments in compliance with federal and state regulations.
- Collaborate with nursing staff, physicians, and other healthcare professionals to coordinate patient care effectively.
- Maintain up-to-date medical records, ensuring all documentation is accurate and compliant with healthcare regulations.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Provide education and support to patients and their families regarding care options and resources available.
- Utilize electronic health record systems such as Epic for documentation and data management.
- Stay informed about changes in regulations related to MDS assessments, DRG classifications, and acute care standards.
Requirements
- Previous experience in hospice care or case management is preferred.
- Strong knowledge of medical records management practices and health information management principles.
- Familiarity with acute care settings, Level II Trauma Centers, and DRG coding is advantageous.
- Understanding of anatomy knowledge relevant to patient assessments.
- Proficiency in using electronic health record systems; experience with Epic is a plus.
- Excellent communication skills, both verbal and written, to effectively collaborate with healthcare teams.
- Strong organizational skills with attention to detail to ensure compliance with regulatory requirements.
- Ability to work independently as well as part of a team in a fast-paced environment.
