Summary
The BRIDGE is seeking a dedicated Community Services Navigator who is passionate about making a positive impact in the lives of others. As a Community Services Navigator, you will play a crucial role in providing support, assistance, and resources to individuals in crisis. You will have the opportunity to work closely with community members, organizations, and resources to ensure that individuals receive the help they need.
The Community Services Navigator is responsible for connecting young adults and families involved in BRIDGE programs or partner organizations to supports and services. The Community Services Navigator is responsible for identifying resources for at-risk families and pursuing other resources to keep families out of the child welfare and juvenile justice systems. Responsibilities also include fielding numerous calls and or electronic requests daily.
Duties and Responsibilities
- Identify and assist young adults and families who are at risk of not meeting their basic needs by obtaining resources to help them lead productive lives and provide a safe environment for their children.
- Communicate with other community professionals and providers to ensure services for young adults and families are coordinated.
- Enter data and maintain electronic files in the designated information systems or databases for program evaluation and tracking purposes; complete regular audits for quality assurance; generate reports on a weekly, monthly, quarterly, and ad hoc basis as needed.
- Ensure situations that will put a young adult or family’s health and safety at immediate risk are handled promptly.
- Conduct interviews using motivational interviewing techniques to assess needs and develop action plans.
- Coordinate other departments in the receipt and processing of requests to ensure there is an open line of communication about the services being received by families.
- Process and coordinate community response requests with other stakeholders.
- Conduct central navigation consultations with young adults and families, ensure all necessary documentation is completed and entered into the database, and make referrals to any additional independent living evaluations deemed necessary.
- Work with young adults and families to ensure family-friendly intake procedures when accessing services and supports.
- Create and maintain relationships with BRIDGE program partners and community partners to provide the best coordination of services for young adults and families.
- Attend BRIDGE, partner programs, and community meetings as needed.
- Responsible for the planning, facilitating, and marketing of young adult and family activities and special events.
- Responsible for supporting young adults and families learning advocacy and leadership skills.
- Perform other duties as needed.
Education and Experience
- Bilingual English and Spanish.
- Associate’s degree in Human Services, Criminal Justice, Education, Social Work, Public Administration, or related field required; bachelor’s degree preferred; or
- Minimum three years of experience (working or lived) with the child welfare system; or
- An equivalent combination of education and experience sufficient to successfully perform the job duties and responsibilities.
Certifications and Licenses
- Valid driver’s license and automobile insurance required.
Knowledge, Skills, Abilities, and Other Characteristics
- Knowledge of available local, state, and federal human service programs.
- High level of knowledge related to the supports and services available to youth aging out of the foster care system.
- Ability to perform, prioritize, and follow through on multiple tasks.
- Strong skills in organization and time management, with a high attention to detail.
- Ability to collect and analyze information and make recommendations.
- Excellent problem-solving and critical thinking skills.
- Strong written and oral communication skills, with the ability to facilitate meetings.
- Ability to build and maintain effective working relationships with families, coworkers, and partner organizations.
- Ability to work with individuals from diverse backgrounds.
- Ability to remain calm during intense situations concerning families’ needs.
- Proficiency in Microsoft Office products (Word, Excel, Outlook)and other forms of technology.
- Ability to work individually as well as collaboratively in a team environment.
- Ability to work independently while providing navigation access to young adults and families at alternate locations.
- Proficiency in English and Spanish (written and verbal) required.
Work Environment
- Work is performed in a casual, open office environment geared towards collaboration. Business attire may be required for some meetings. All necessary office equipment is provided.
