Our organization assists people to rebuild skills they were once able to do independently before terrible tragedy struck in the form of a car accident, stroke, assault, fall, or before they were struck with a debilitating disease.
Overview:
As the Director of Community Services, you will lead and oversee our Service Coordination Team, Health Home, Social Care Network, and Independent Living Skills Services. You will play a pivotal role in ensuring these teams are supported, efficient, and aligned with our organizational budget and program goals. Your leadership will be instrumental in enhancing the quality of life for brain injury survivors and people with disabilities within our community.

The ideal candidate for the Director of Community Services position will bring proven experience in program management, particularly in healthcare or non-profit/community services settings. They should have a track record of successfully overseeing and implementing programs that serve individuals with diverse needs, with a deep understanding of how to manage resources effectively and achieve program goals.
In addition to their program management experience, the candidate will demonstrate strong leadership abilities. They will be capable of inspiring and motivating their team, fostering a positive and collaborative work environment. The Director will be someone who leads by example, helping to cultivate a culture of professionalism, teamwork, and dedication to the organization's mission.
The role requires excellent organizational and communication skills. The ideal candidate will be able to handle multiple priorities simultaneously, maintain a clear focus on goals, and ensure that all team members are aligned and informed. Their ability to communicate clearly and effectively with internal teams, external partners, and clients will be crucial in ensuring the success of the programs under their oversight.
Additionally, a strong understanding of regulatory requirements and best practices in service coordination and community-based services is essential. Knowledge of NYS Department of Health Medicaid Waiver programs a plus!

Preferred Education & Experience
· Bachelor's degree in Social Work, Public Health, Human Services, or related field (Master’s degree preferred).
· Proven experience in program management, preferably in a healthcare or community services setting.

· Understanding of regulatory requirements and best practices in case management/service coordination and community-based services

The salary range for this role is $50,000 - $54,000 Salary offers are commensurate with education & experience.
