Our mission is to guide brain injury survivors, individuals with disabilities, and their families, by ‘bridging the gap’ between surviving and living lives of independence with dignity and hope. Our organization assists people to rebuild skills they were once able to do independently before tragedy struck in the form of a car accident, stroke, assault, fall, or before they were struck with a debilitating disease.

 

Here's What We’re Looking for in an Independent Life Skills Trainer:

We are currently seeking a dedicated, self-motivated, and organized candidate to work closely with brain injury survivors and individuals with disabilities in their homes across Ontario, Monroe, and surrounding counties. Responsibilities include assessing their skills and developing personalized goal plans to enhance independence in household management, budgeting, health management, meal preparation, grocery shopping, planning, and organization. The Independent Living Skills Trainer (ILST) will dedicate approximately 4 hours per week per participant, teaching effective strategies and techniques to achieve these goals. This role offers significant autonomy, including opportunities for remote work. Ideal candidates must thrive independently, demonstrating strong discipline to meet deadlines consistently.

 

The Life Skills Trainer Should Have Experience with and The Ability To:

  • Conduct ADL/IADL skill assessments
  • Develop goal plan with the participant, monitor effectiveness and revise as needed
  • Teach participant strategies and techniques to implement in their daily life and home to increase organization and independence in daily tasks.
  • Coordinate with the person’s team of providers as needed to foster positive communication and ensure the person’s needs are being met.
  • Support and encourage the participant to increase problem-solving, advocacy, decision-making skills, independence, and participation in community activities and services.
  • Meet Department of Health and Bridge’s deadlines for documentation submission.
  • Maintain all paperwork and reports as required by the NYS DOH programs and Bridges

Organization: Life is always changing, and this is often the case with the people you will be working with on your caseload. No day will ever be the same. Expect to jump from task to task depending on the needs of your caseload and upcoming deadlines. To do this well, we will need you to be great at planning, prioritizing, multi-tasking and doing what it takes to get stuff done!

Team Attitude: We are all on the same team and when one of us succeeds, we all do. As a small organization we depend on each other to pitch in on things that may not be in your job description.

Passion, Resilience and Persistence: Working with people who are dealing with significant loss and chronic health conditions is not easy. It takes passion to help others achieve their goals; and resilience and perseverance to get through some of the days that will be demanding, but in the end makes your work meaningful and rewarding!

 

Preferred Education & Experience

  • BA/BS degree in social work, psychology, gerontology, or another related field.
  • 2 years’ experience completing independent living skills functional assessments and goal plans  with people with brain injury, disabilities  and/or seniors.
  • Knowledge of and accessibility to community resources.
  • Excellent organizational skills and attention to detail
  • Adept at multi-tasking
  • Strong desire to help make people’s lives better

 

The pay range for this position is $19 to $25 per hour. Salary offers are commensurate with education & experience.
