Case Manager - IFP
$21.07 - $27.08 per hour

PURPOSE and POSITION OVERVIEW
IFP provides intensive home based services to families referred by the Department of Children and Families (DCF) to mitigate safety factors that led to abuse or neglect of clients.
DUTIES and RESPONSIBILITIES
- Provide direct community- based case management services to children and families.
- Coordinate care and initiate referrals between all service providers.
- Maintain client records.
- Performs any other department or agency related duties or special projects as directed by supervisor.
QUALIFICATIONS and COMPETENCIES
Bachelor's degree required, with major in human services preferred.
Ability to work a flexible schedule.
Sensitivity to diverse cultures and language competencies.
Knowledge of behavioral health issues and an understanding of home based services.
Good communication and documentation skills.
Computer literacy.
Ability to prioritize and organize schedules/demands.
CPR/First Aid certified or willing to become certified.
Valid driver's license and access to reliable transportation on a consistent basis.
Bridges is STAR-LRP AND HRSA Approved, and some positions may be eligible for loan repayment through these programs.
