PEER CASE Coordinator

Position Descriptions:

The Peer Case Coordinator will be an individual with lived experience of justice system involvement who uses their experience to assist in fostering successful re-integration of individuals from incarceration to the community. The Peer Case Coordinator will assist in the implementation of service coordination outlined by the County Re-entry Task Force programmatic guidelines and peer mentoring appropriate for individuals transitioning home from state incarceration, those with justice system involvement, and their families to help reduce their criminal involvement and recidivism and overcome barriers to success in their family, school, community, employment, health, and life.

DUTIES AND RESPONSIBILITIES:

· Mentor, motivate, inspire, and empower clients by establishing a trusting relationship with client

· Assist clients in navigating community resource systems through appointment accompaniment.

· Participate/Co-facilitate necessary programs when appropriate including peer driven ‘Welcome Home’ group

· Assist in assessment and development of service coordination plans in accordance with individual participant goals

· Assist in the provision of information and referral, follow-up/monitoring

· Assist with removal of barriers to help participants access needed services including housing, benefit coordination, treatment, employment/vocational training, etc

· Provide guidance and mentoring where appropriate

· Maintain accurate and timely documentation

· Participate/Facilitate in the coordination of community outreach and educational events

· Attend required meetings and trainings

· Other related duties as assigned

REQUIRED QUALIFICATIONS:

· High School Diploma or equivalency

· Lived experience in the criminal justice system (Candidates should have a personal history of incarceration and successful re-entry into the community following incarceration).

· A willingness to work with diverse communities

· Good verbal and written communication skills in English

· Computer literate and proficient with Microsoft Word and Google Chrome

· Bilingual English/Spanish highly preferred

Certificates, licenses, and registrations required: Peer Navigator certification required upon hire

Job Type: Full-time

Pay: $19.00 - $22.00 per hour
