POSITION DESCRIPTION: BRIDGES operates a variety of programs guided by the philosophy of Independent Living to provide services and supports for individuals with disabilities so that they may experience autonomy in all areas of living.
BRIDGES is the joint employer - Agency with Choice Financial Management Services model, that allows Veterans to recruit, interview, hire and train their own direct care worker. The Person Centered Counselor serves as an advocate and coach to a Veteran, caregiver(s) and/or authorized representative and ensures compliance of BRIDGES’ operational and contractual policies, procedures, fiscal and personnel mandates are satisfactory.
The VDC Case Manager holds the sole responsibility for conducting intakes for community based home care for Veterans all across the Hudson Valley . The VDC Case Manager will make themselves available to the veteran as needed and meet them where they are in the community.
QUALIFICATIONS:
- A four-year degree preferred and three years paid or volunteer experience in Human Services or completion of at least one year of post-secondary education and seven years paid or volunteer experience in human services.
- Good interpersonal skills; treats people with respect and sensitivity for cultural differences.
- Maintain strong work ethic and strict adherence to code of ethics, standards of conduct, and confidentiality requirements.
- Must be able to travel locally and regionally for Veteran support services and meetings when needed.
- Ability to provide exemplary customer service.
- Must be able to maintain solid documentation.
- Working knowledge of Microsoft Office Software, including Excel, Word and Powerpoint.
- Ability to become proficient on other software products as utilized by the position.
- Excellent interpersonal, organizational, written and oral communication skills; able to interact with all levels of associates in a respectful, confidential manner.
- Effective and collaborative team player.
- Must complete all required training specific to the program.
- Maintain strict confidentiality and protects Agency operations by keeping information confidential and secured.
- Educate the Veteran on the Veteran Directed Care Program.
- Work with the Veteran to assess, identify, coordinate and monitor supports and services on an ongoing basis.
- Conduct initial assessments and reassessments with the Veteran.
- Support the Veteran to create and manage their spending plans.
- Monitor all Veteran Directed Care spending along with the Veteran and the VA Medical Center.
- Maintain monthly communication with the Veteran/Representative.
- Conduct enrollments for direct care workers.
- View time records of direct care worker for accuracy to ensure timely processing of payroll.
- Prepare and present narrative and statistical reports as required.
- Understanding of the Person Centered Care approach.
- Maintain accurate record keeping, note taking, and data entry.
- Facilitates consistent learning and development on Veteran Directed Care.
- Establish and maintain a strong attention to detail, ensuring work is accurate and complete, and adhere to all established deadlines.
- Demonstrates self-direct and the ability to prioritize and accomplish job responsibilities.
- Bilingual preferred but not required.

Pay: $24.00 - $27.00 per hour
