This is a dayshift role, 7 : 00am to 7 : 00pm. The NP / PA will round on patients in the morning and provide cross-coverage on admissions until the end of their shift. There is a minimum requirement of two shifts per month. We are located on the CDH Main Campus at 30 Locust St, Northampton, MA 01060. The NP / PA will support physicians, collaborate with consultants, nurses, case managers, rehab providers, and others to provide high-quality, efficient care for the Hospitalist Service patients. • Caring for patients on the med / surg floors and acting as first response with supervising physician
• Completing initial patient evaluations, inclusive of obtaining a history, completing medication reconciliation, performing an exam, reviewing diagnostic testing, and documenting an H&P as well as entry of admission orders
• Monitoring patient progress and coordinating treatment plans with supervising attending, consultants, and providers in other disciplines
• Documenting subsequent encounters through completion of progress notes
• Performing procedures in accordance with clinical privileges
• Discharging the patient from the hospital, including :
• Performing discharge medication reconciliation
• Coordinating discharge care plan, including follow-up with Primary Care Provider and / or subspecialist as needed to ensure safe care transitions
• Providing education to patients, and family members, regarding their illness, medications, nutrition, diagnostic tests, procedures, care plan, and post-discharge care
• Triaging patients from CDH ED as well from outside hospitals
• Collaborating with nurses, hospitalists, and other providers to accept patients, as appropriate, from CDH ED, outside hospital EDs, and outpatient clinics

• Master's degree in nursing or physician assistant studies required.Experience :
• 2-3 years of inpatient NP or PA experience preferred.Certification :
