About the Role:
The Child Welfare Case Manager plays a vital role in delivering case management services, individual and family counseling, training, and supervision of foster families. They work under the guidance of the Program Manager to develop and implement individual treatment plans for troubled children and their families.
Responsibilities:
- Reviews referrals and matches children with suitable foster families.
- Places children in state-licensed or certified foster homes.
- Obtains required documentation for children in care.
- Provides regular contact and visits to children and foster homes.
- Prepares treatment plans and progress reports for assigned children.
- Coordinates visitation between children and biological parents/siblings/relatives.
- Liaises with placing agencies, schools, and community organizations.
