We are seeking a skilled and compassionate Support Coordinator to join our team at BrightSpring Health Services.
The ideal candidate will possess strong interpersonal and communication skills, with the ability to work independently and as part of a team. Our Rehabilitation Program is designed to provide individuals with brain injuries, spinal cord injuries or stroke/CVA with a continuum of care that promotes greater health through the recovery process.
In this role, you will be responsible for assisting patients in achieving their individualized treatment plan goals, ensuring their safety and living comfort, and fostering independent functioning. You will also have the opportunity to participate in community reintegration activities under the direction of the clinical and/or residential treatment team.
To be successful in this position, you must be able to demonstrate effective and professional interpersonal, verbal, and written communication skills, as well as strong organization skills and attention to detail.
