Location : Thome PACE

Overview

The Occupational Therapist will evaluate and reassess participants referred by the Interdisciplinary Team, providing specialized therapy services to improve participants' functional abilities. Working under the direct and indirect supervision of the Restorative and Recreation Therapy Supervisor, the OT is expected to independently apply advanced Occupational Therapy knowledge and skills in patient care. The therapist will deliver services both in the day center and in participants' homes, as needed, tailoring therapy interventions to meet individual needs and goals. This role is focused on promoting independence, enhancing daily living skills, and maximizing overall participant well-being.

What you’ll do

Patient Assessment & Evaluation :

  • Conduct initial assessments and reassessments, communicating any changes to the team promptly.

Treatment Planning & Implementation :

  • Develop individualized treatment plans, set measurable goals, and provide interventions.
  • Document treatments and progress in participant medical records.
  • Care Coordination :

  • Coordinate 24-hour care and assess the need for adaptive medical equipment.
  • Collaborate with the Home Care Coordinator to arrange the necessary equipment and review assessments for discharge.
  • Training & Supervision :

  • Train family, friends, and staff on treatment plan components and supervise aides.
  • Team Collaboration :

  • Participate in Interdisciplinary Team meetings, contributing to care planning and case conferences.
  • Quality Improvement & Compliance :

  • Engage in Quality Improvement processes and ensure compliance with policies and regulations.
  • What it takes

  • Graduate of an ACOTE-accredited Occupational Therapy program.
  • Licensed Occupational Therapist in Michigan.
  • CPR certified.
  • Minimum of two (2) years of experience as an Occupational Therapist.
  • At least one (1) year of experience working with frail and elderly populations.
  • Strong assessment and intervention skills, particularly for elderly or frail individuals.
  • Ability to create and implement therapy plans to improve daily living skills and independence.
  • Excellent communication and interpersonal skills for working with patients, families, and teams.
  • Compassionate, patient-centered approach to care.
  • Ability to maintain accurate, compliant patient documentation.
