🎯 What You’ll Do in This Role:
The Clinical Pharmacist at Thome PACE plays a key role in optimizing medication therapy for older adults through participant-centered clinical services. This role supports the Interdisciplinary Team by identifying and resolving drug-related issues, ensures compliance with Part D regulations, and promotes cost-effective medication use. You'll also collaborate with hospitals, pharmacies, and leadership to close care gaps, enhance outcomes, and support continuous quality improvement.
Primary Responsibilities:
👥 Participant Care
- Complete assessments to support care planning and discharge
- Review and monitor medication therapy for safety and effectiveness
- Communicate changes and documents in the care plan
- Counsel participants, families, and caregivers on medications
- Provide education to staff, students, and healthcare professionals
- Ensure timely and accurate documentation
- Promoting a positive participant experience
đź’ĽÂ Medication Management
- Conduct comprehensive medication reviews and utilization initiatives
- Support compliance with Medicare Part D regulations
- Improve safety, quality, and cost-effectiveness of medication use
- Oversee support staff and delegate tasks as needed
- Monitor prescribing trends and implementing pharmacy initiatives
- Stay current with literature and implement best practices
- Perform audits and support reporting for leadership
- Assist with data collection for Part D bids
✅ What You’ll Need:
- Active Michigan Pharmacist license and Controlled Substance license
- Doctor of Pharmacy (PharmD) required
- PGY1 Residency in Ambulatory Care or Geriatrics preferred
- Leadership experience and strong collaboration skills preferred
