Position Summary: This position acts as support to the Community Transition Liaison

Job Summary:
Bristol is seeking a highly motivated, organized and personable individual to join our Community Transition Liaison Program Team (CTLP Team). The CTLP Team is explicitly dedicated to support consumers who want to transition from a nursing facility to the community. The Case Assistant acts as support to the Community Transition Liaisons.
Job Duties:
Duties will include accompanying the Liaison on nursing facility visits throughout the Greater Fall River, Taunton, and Attleboro areas and meeting with residents and staff to promote the program and identify residents in need of transitioning to the community. Additional duties include, gathering documentation needed to assist consumers in applying for public benefits, completing, and submitting housing applications and conducting clerical duties as needed by the Liaison.
Education & Experience:
- High School diploma with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting.
- Excellent interpersonal skills,
- good computer skills,
- organizational and time management skills
- an ability to multi-task.
- Bilingual Portuguese, Spanish, and English candidates preferred.
Schedule: 35-hour work week, 9:00 a.m. – 5:00 p.m., Monday – Friday.
