Bristol Aging and Wellness, Inc. (Bristol) located in Fall River, MA, is a private, non-profit agency that provides community-based support services. We offer a comprehensive benefits package.
Bristol is hiring a Community Transition Liaison. This position will be working as part of the Community Transitions Liaison Program Team, explicitly dedicated to supporting consumers who want and need assistance to plan a transition from a nursing facility to the community. This position acts as the on-site point of contact for residents, families, and skilled nursing facility (SNF) staff related to transitions from SNF to the community. The Community Transitions Liaison will complete weekly site visits to nursing homes in the Greater Fall River, Taunton, and Attleboro communities. The Community Transitions Liaison will also be working with a variety of community agencies to collaborate on transitions to the community. The Community Transitions Liaison will work directly with a CTLP Case Assistant who will provide clerical support.
Bachelor’s degree in social work, human services, nursing, psychology, sociology or a related field preferred OR a bachelor’s degree in another discipline and demonstrates experience and / or strong interest in the field of human services via previous employment, internship, volunteer activity and / or additional studies. Must possess knowledge of long-term care, case management, discharge planning, community resources, programs, and benefits to help support an individual’s transition from an institutional to a community setting. Excellent oral and written communications skills, ability to effectively relate to a wide variety of personalities, basic computer skills, organizational and time management skills and ability to multi-task.
35-hour work week with flexible scheduling options following introductory period.
Bi-lingual, Portuguese, or Spanish preferred.
