Responsibilities:
- · Provide one-on-one patient care in the patient's home, following a detailed care plan provided by the Nurse Care Manager and/or Physician
- · Perform skilled nursing tasks such as administering medications, wound care, and monitoring vital signs
- · Collaborate with the patient's healthcare team to develop and implement an effective care plan
- · Educate the patient and their family on proper care and treatment method
- · Maintain accurate and up-to-date patient records
- · Communicate effectively with patients, families, and healthcare professionals
Qualifications:
- · Registered Nurse (RN) license in IL
- · Minimum of 2 years of nursing experience, preferably in home health or hospice care
- · BLS/CPR certification
- · Strong clinical skills and knowledge of medical equipment and procedures
- · Excellent communication and interpersonal skills
- · Compassionate, patient-centered approach to caregiving
- · Ability to work independently and manage time effectively
