Brockton Home Health Care agency is looking for abi-lingual Community RN / Case Manager to manage and support our AdultFamily / Foster and Home Health Care services to make sure the our programs runssmoothly.
Jobdescription
- Responsible for overall supervision of all HomeVisits RN's;
- Completing the MDS-HC and other clinicalassessments;
- Developing and reviewing on an ongoing basiseach member's AFC / GAFC plan of care;
- Training, evaluating, and supervising AFC / GAFCcaregivers in conjunction with Program Director;
- Reviewing the physician summary;
- Monitoring the health status of all members toensure that all needed AFC / GAFC is properly delivered;
- Reporting changes in the health status of anymember to the member's physician;
- Periodically reviewing AFC / GAFC caregiver logs;
- Conducting admission on-site visits for newmember;
- Completing a nursing progress notecorresponding with each on-site visit or encounter;
- Submitting a semiannual health-status report tothe member's primary care physician;
- Planning for an implementing discharges fromthe AFC / GAFC program;
- Conducting an orientation for each AFC / GAFCcaregiver before the AFC caregiver begins personal care;
- Providing ongoing training to AFC / GAFCcaregivers on health and aging
- Any other office duties that may be assigned byProgram Directors.
