What is AFC? Adult Foster Care is a MA Health funded program for adults with disabilities and elderly people who can't live alone safely. We assist people ages 16+ who need assistance due to aging, medical conditions, mental illness, developmental and/or physical disabilities. Members are matched with approved families that provide lodging and personal care services in their home. At times, families serve as AFC caregivers for their loved ones.
CARE MANAGER Job Summary
We are currently seeking a full- or part-time on-site Care Manager for our Adult Foster Care Program to provide psycho social and counseling assistance for the program - up to $22/hour. This position is an integral part of a multidisciplinary team. This role completes visits at the caregiver's home monthly or bi-monthly and ensures continuity of care for the member placed with the caregiver.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following, other duties may be assigned.
Conduct ongoing psycho-social assessment and evaluation of a member’s appropriateness for AFC, as needed;
Evaluate, support, and train the AFC caregiver;
Support and train the caregiver in all aspects of psychological, social, safety and independent activities of daily living. 3. Provide continued training to all participants and caregivers to insure safe practices
Ensure implementation of the AFC plan of care; help participants to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and participant's part in creating them.
Conduct on-site visits with each member at the qualified setting: for Level I, bimonthly (alternating with the bimonthly visit by the registered nurse); for Level II, monthly;
Complete a care-management progress note corresponding with each on-site visit or encounter and upon significant change;
Conduct regular, periodic evaluations and assessments, at least annually, to ensure that each qualified setting where AFC is provided meets the requirements of 130 CMR 408.435;
Make referrals to appropriate service providers if a member requires health or social services other than AFC;
Provide timely assistance and respond to urgent or emergency needs of the member; Compile and maintains participant records as required according to policy and procedures, to include leaves of absences assistance and accountability.
Develop an emergency backup and personal care contingency plan for each member, that includes an alternative care plan for the member if the AFC caregiver is temporarily unavailable or unable to provide care.
Implement Commonwealth of Massachusetts regulations (130 CMR 408.000) that are published as Subchapter 4 of the Adult Foster Care Manual. 12. Implement PRIDE Policy and Procedures to assist participants in establishing program goals and positive outcomes.
Prepare required reports to insure AFC Program meets all requirements and regulation and inform Director on any conditions in participant situations or local agencies which may impact the provision of service.

EDUCATION and/or EXPERIENCE Qualifications :
Bachelor’s degree and two years’ clinical experience in the care of elderly or disabled persons or
Bachelor's degree from four-year College or university and one to two years related experience and/or training
Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts. Ability to apply concepts such as banking record-keeping, adding receipts, doing budgets, fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
