POSITION SUMMARY :
The Reentry Project Manager (RPM) functions as a core member of a collaborative care team that involves the patient, the patients' medical provider, psychiatry, nursing, and community health workers. The RPM conducts in reach at Plymouth County Correctional Facility (PCCF) at least two days a week, while working out of BNHC's brick-and-mortar clinic for the remainder of the work week. The RPM will be tasked with administrative responsibilities related to project management, as well as the clinical responsibilities of a Behavioral Health Clinician for individuals reentering from incarceration. Project management tasks will include program staffing, scheduling, staff training and supervision, and data collection and reporting. Clinical tasks include consultation / assessment and bio-psychosocial evaluations, interventions, counseling (group and individual), and support and linkage services, advocacy, education, information to ensure adequacy of health care, increasing access of services, enhancing physical and psychological well-being and facilitating patient self-care. The Reentry Project Manager works as part of a diverse team in a multicultural Community Health Center to coordinate and deliver cost-effective behavioral health interventions in a primary care setting for justice-involved individuals.
