Job Summary
Community Support Specialist provides an array of support services delivered by community-based, mobile Community Support Specialist. CSS are directed towards children, adolescents and families and will vary wit respect hours, type and intensity of services, depending on the changing needs of each individual. The purpose of CSS is to provide specific, measurable, and individualized services to each person served. CSS should be focused on the individual's ability to succeed in the community; to identify and access needed services; and show improvement in school, work and family and integration and contributions within the community.
Education
Community Support Specialist should have at least a Bachelors' Degree in Psychology, Sociology, Social Work or a related field. Community Support Specialist should have at least 1 year experience in the mental health or related field.
Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation.
- Monitor vital signs and report any changes or concerns regarding the client’s health status.
- Develop and implement care plans tailored to individual client needs.
- Provide patient monitoring to ensure safety and well-being in a home or community setting.
- Maintain a clean and safe environment for clients by adhering to hygiene standards.
- Support individuals with disabilities by promoting independence and facilitating access to community resources.
- Collaborate with healthcare professionals to ensure comprehensive care for clients.
- Document care provided and any changes in the client's condition accurately.
Requirements
Community Support Specialist should be able to work independently and within a community setting to perform all duties community-based environments. Community Support Specialist should provide services that are person-centered and focus on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in home, health, purpose and community. Community Support Specialist must be willing to work from a self-established schedule and stay current with all documentation and agency required paperwork. All documentation and paperwork must be completed in a timely manner.
-Must report and work well with the Clinical Director and Office Manager.
-Must work well with the assigned Mental Health Therapist.
-Must apply for PCSS within 30 days of hire from DMH.
- Knowledge of HIPAA regulations and confidentiality practices.
- Maintain confidentiality and adhere to HIPAA regulations.
- Collaborate with multidisciplinary teams to ensure the well-being of clients.
- Strong communication skills and the ability to work compassionately with diverse populations.
-Must complete CPR.
-Must complete drug screen.
-Must complete background check.
-Must be willing to travel.
-Must be willing to market in the community and surrounding areas.
Community Support Specialist will provide the following services:
- Identification of strengths which will aid the individual in their recovery and the barriers that will challenge the development of skills necessary for independent functioning in the community.
- Individual therapeutic interventions with a beneficiary that directly increase the acquistion of skills needed to accomplish the goals set forth in the Individual Service Plan.
- Monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals.
- Psychoeducation on the identification and self-management of prescribed medication and communication with the prescribing provider.
- Direct interventions in deescalating situations to prevent crisis.
- Assisting an individual in accessing needed services such as medical, social, educational, transportation, housing, personal care, employment and other services that may be identified in the CSS plan as components of Health, Home, Purpose and Community.
- Assisting the beneficiary and natural supports in implementation of therapeutic interventions outlined in the Individual Service Plan.
- Relapse prevention and disease management strategies.
- Psychoeducation and training of family, caregivers, and others who have a legitimate role in addressing the needs of the individual.
- Facilitation of the Individual Service Plan which includes the active involvement of the beneficiary and the people identified as important in the person's life.
