JOB DESCRIPTION
The ideal candidate will use their clinical knowledge, communication skills, and marketing abilities to initiate dialogue with referral sources, families, and prospective patients to help explain and educate on home health and hospice services. Our Clinical Liaisons are home health and hospice experts and serve as a resource to referral sources to ensure extraordinary patient care and customer service.
RESPONSIBILITIES
- Successful Liaisons will educate the community about the services provided by Buena Vista Home Health and Hospice and will develop and maintain positive relationships with families, caregivers, medical professionals and the communities we serve
- Coordinates with physicians and referral sources (as appropriate) regarding home health and hospice plans of care.
- Reviews clinical documentation to help determine home health and hospice appropriateness.
- Reviews patient medical charts and understands pharmaceuticals, test results, therapy evaluations, pre-existing conditions, and has a general medical knowledge of the patient
- Communicates with patients, families and caregivers regarding home health and hospice services.
- Conducts thorough patient assessments to identify patients for potential admission into the system
- Develops educational and community relations plans in consultation with agency leadership
- Participates in Clinical Program Development as identified through market demand
- Maintains an in-depth knowledge of compliance with all local, state and federal laws related to hospice regulations and care plan development techniques.
QUALIFICATIONS
- Holds current unencumbered license as an LVN or RN.
- Experience in hospice marketing strongly preferred.
- Current Basic Life Support Certification
- Minimum one year recent professional nursing experience
- Valid California Driver’s License

Pay: $70,000.00 - $90,000.00 per year
