SUMMARY
The Case Manager advocates for and provides holistic, client-centered case management services to support clients in achieving independence and self-sufficiency. This role involves conducting comprehensive assessments, creating individualized service plans, and providing resources and referrals to address mental health, housing, employment, and other needs. The Case Manager also monitors progress, supports crisis intervention, and promotes stability, in alignment with Buffalo City Mission’s trauma-informed care approach.
ESSENTIAL FUNCTIONS AND TASKS

Client Assessment and Planning
Conduct comprehensive assessments to identify client needs, strengths, barriers, and goals.
- Develop and regularly update Individualized Service Plans (ISPs) to address clients' long-term and short-term objectives.

Facilitate intake processes for clients interested in entering transitional or emergency housing programs.
Service Coordination - Provide holistic, comprehensive case management services, connecting clients to housing, healthcare, employment, education, and other essential resources.

Facilitate and document referrals, linkages, and follow-ups with internal and external service providers.
Case Management and Advocacy - Advocate for clients to ensure equitable access to resources and services.
- Support clients with goal setting, progress tracking, and addressing barriers.

Provide supportive counseling, tenant education, and financial management support to clients.
Crisis Intervention - Identify and address client crises, including mental health emergencies, housing instability, or substance use issues.

Implement de-escalation techniques and ensure proper referrals to crisis services.
Compliance and Documentation - Maintain detailed, accurate, and confidential client records in compliance with federal, state, and organizational standards.
- Submit required reports, including progress notes, outcome evaluations, and documentation of services provided.

Participate in audits to ensure quality and compliance.
Collaboration and Teamwork - Work collaboratively with internal teams, community partners, and external agencies to enhance client outcomes.

Attend and actively participate in team meetings, case conferences, and training sessions.
Professional Development - Stay updated on industry best practices, community resources, and emerging trends in case management.
- Participate in professional development opportunities, including trauma-informed care and crisis intervention training.
Additional Responsibilities
- Assist in Code Blue efforts and other emergency housing initiatives as needed.
- Facilitate client education classes and other programming to support client progress.
- Represent the organization in a professional and Christ-like manner when interacting with clients, coworkers, and external stakeholders.
QUALIFICATIONS
- In the midst of a mature walk with Jesus Christ and able to articulate belief.
Educational Background:
- Associate Degree in Human Services or related field required; Bachelor’s Degree preferred.
Professional Experience:
- Minimum of 1-3 years of experience in case management, social services, or a related field.
- Experience working with homeless or vulnerable populations preferred.
Other Qualifications:
- Knowledge of mental health, chemical dependency, domestic violence, and community resources.
- Bi-lingual or bi-cultural skills preferred.
Technical Skills:
- Proficiency in Microsoft Office and case management software.
- Familiarity with trauma-informed care and client-centered approaches.
