What are the responsibilities and job description for the Supportive Housing Case Manager position at Buffalo Federation of Neighborhood Centers?
Position Purpose: The Supportive Housing Case Manger assists individuals with mental health diagnosis to obtain safe, appropriate housing and to maintain residential stability. Services provided may include identifying appropriate housing options, coordinating moves, coordinating services with other providers, determining client eligibility for housing programs, facilitate resolution of landlord/tenant conflict, and assisting client to develop daily living skills.
Duties and Responsibilities
- Provides advocacy and linkage to behavioral health, medical and community services.
- Engage program participants for purposes of developing rapport and building trust;
- Locate appropriate rental units in neighborhoods; assist with contacting landlords and negotiating lease agreements consistent with program guidelines and approved budget caps.
- Conduct apartment set-ups, especially focusing on assisting consumers with becoming acclimated to their new environment. Continuously monitor and assess consumers adjustment to their apartment and the surrounding community.
- Assist participants to develop or enhance skills necessary to live successfully in the community, including awareness of rights, self-advocacy, and verbal negotiation, problem-solving, money management, housekeeping, personal hygiene, community mobility, shopping, nutrition management, leisure time activities.
- Assist in resolving consumer conflicts with neighbors and/or landlords.
- Maintain case records and all required documentation consistent with OMH, ECDMH and agency guidelines.
- Assists with admission and orientation of new residents; assure timely processing of all paperwork.
- Ensures continuity of residents’ entitlements through timely submission of required forms and documentation
- Responsible for resident discharge process including all applicable paperwork.
- Develop effective working relationships with agency employees and other service providers.
- Regularly provides supervisors of pertinent information regarding the status of program participants.
- Know and support agency and program vision, mission, goals, objectives, policies, and plans.
- Participate in other agency activities as a team member.
Requirements
- High School Diploma and five (5) years full time work experience working in the Human Service Field.
- One (l)year paid experience in direct service position to persons with handicapping Disabilities preferably in a residential setting preferred
- Must be computer literate (Word, Excel, Outlook);
- Must have use of an automobile and valid driver’s license with evidence of good driving record.
- Must be able to work some evenings and weekends.

Pay: $40,000.00 - $45,000.00 per year
