Summary: Responsible for day-to-day operation of the Transitional Housing Program. Provides direct care services to residents in the Housing program, such as life skills and transitional support services, case management and activities. Responsible for managing the house, safety of residents and cleanliness of facility.
Duties
- To provide housing case management services and support to adult individuals who are experiencing homelessness, with a goal of placement into permanent housing.
- Performs the daily operations of the Transitional Housing program including the processing of intakes and referrals for admission into the house as identified in shift responsibilities list.*
- Responsible for meeting with residents in individual and group settings regularly for service planning, life skills training, and activities.
- Provide crisis management support to residents in urgent situations.
- Utilize motivational interviewing techniques to encourage client engagement and progress.
- Maintain accurate documentation of client interactions and progress. Timely data entries of weekly case management notes, housing updates, and other documentation into programs electronic record system and resident paper charts.
- Meet with residents to address barriers and successes to assist in strengthening their capacity for long-term self-sufficiency
- Assists with necessary outreach and engagement activities to include clients, community providers, landlords, and other service providers.
- Participate in regular team meetings to discuss case progress and share best practices.
Qualifications
- Experience in human service field is highly desirable.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail.
- Ability to work independently as well as part of a collaborative team environment.
- Commitment to ongoing professional development and training in case management practices.
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
Expected hours: 40 per week
