Duties of Position:
Maintains the quality of client care in accordance with the Agency’s objectives and policies through planning, coordination, and implementation and evaluation of the home care services provided
POSITION RESPONSIBILITIES:
- This job requires the RN to work out of the office for all paperwork, this is not a home based position. Additionally all completed paperwork must be handed in within 24 hours of the visit to the home.
- Provides supervision of Personal care Aides to ensure the quality of services and maintenance of standards.
- Reviews and investigates the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients.
- Directs staff toward the appropriate use of community resources.
- Appraise staff performance during their probationary period and annually with the Director of Patient Services, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.
- Identifies and evaluates client needs and appropriateness of home care by reviewing the initial screening form before assignment of direct care staff.
- Assists with the orientation of new professional staff to familiarize them with Agency policies and procedures.
- Sees clients in the home setting as needed.
- May be scheduled in the office to complete PPD's or Physicals as needed.
- Completes client’s re-hospitalization referral/admission forms as needed
- Performs other duties as directed by the Administrator, Office Manager, Owner or Director of Patient Services.
Job Conditions:
- In-office role may involve some clinical and occasional lifting of medical supplies and/or charts.
- Travel is required to and from client’s home when supervision of care is scheduled.
- The ability to communicate well, both verbally and in writing in English is required.
EQUIPMENT OPERATION:
- Must have knowledge of how to use any HOYER type.
Qualifications:
- Must be licensed in NY State as a Registered Professional Nurse.
- Must be mature, professional and willing to be flexible to change directions without notice due to the needs of the business.
- Ability to meet physical requirements of the position when required: sitting, standing, lifting, squatting, stooping, kneeling, bending.
- Ability to understand and react to emergency situations.
- Must have a valid NYS Driver’s License and access to a car to get to/from client’s homes.
- Must pass the OMIG verification.
