What are the responsibilities and job description for the Congestive Heart Failure (CHF) APP position at Buffalo Medical Group?
Buffalo Medical Group is seeking a Congestive Heart Failure advanced practitioner who will be responsible for managing patient care, coordinating with other healthcare providers, and ensuring that patients receive the highest quality of care possible. This provider will play a critical role in early diagnosis, triage, and management of patients with congestive heart failure (CHF), working collaboratively with cardiology, primary care, nephrology, and pulmonology to optimize patient outcomes.
This provider will need to see patients at our Williamsville, downtown, Orchard Park and Lockport locations.
Key Responsibilities:
Patient Evaluation & Diagnosis
- Perform comprehensive assessments of patients at risk for or diagnosed with CHF.
- Triage patients efficiently to facilitate early diagnosis and intervention.
- Utilize evidence-based guidelines to formulate individualized treatment plans and care pathways.
Collaborative & Multispecialty Care Coordination
- Work closely with cardiology, primary care, nephrology, and pulmonology to ensure a holistic approach to CHF management.
- Coordinate and order imaging studies, laboratory tests, and diagnostic evaluations as needed.
Patient Education & Support
- Provide comprehensive education to patients and families on CHF management, medication adherence, lifestyle modifications, and symptom recognition.
- Empower patients to actively participate in their care and improve self-management strategies.
Clinical Management & Treatment Planning
- Optimize medication regimens, including initiation and titration of guideline-directed medical therapies (GDMT).
- Identify patients who may benefit from advanced heart failure therapies.
Quality Improvement & Program Development
- Participate in CHF program development to enhance early detection and treatment pathways.
- Contribute to quality improvement initiatives, protocol development, and performance metrics tracking.
- Engage in ongoing education to stay updated on CHF management guidelines and best practices.
Minimum Qualifications:
· Prior experience in cardiology, heart failure, internal medicine, or a related specialty preferred.
· Strong clinical skills in evaluating, diagnosing, and managing CHF patients.
· Experience caring for adults with serious illness is preferred
· Previous EMR experience, particularly epic, is preferred
· Must be able to perform any clinical or clerical duty as assigned skillfully
· Must demonstrate strong verbal and communication skills including ability to present information in a group setting
· Must demonstrate the ability to handle stressful situations appropriately
· Must be able to work flexible hours as needed
· Must demonstrate ability to work independently and as part of the team
· Must be able to travel to all BMG locations
Job Type: Full-time
Pay: $90,446.40 - $135,669.60 per year
