What are the responsibilities and job description for the APP-Primary Care position at UBMD Internal Medicine?
UBMD Internal Medicine is looking to add a Full Time Advanced Practice Provider (APP) to our Primary Care Team!
Position Summary:
The APP will provide preventive, chronic, and urgent care care to a panel of patients and will support physician providers at the Youngs Road practice site. The APP may also be asked to support activities of the Department as assigned by the Medical Director and/or Collaborating Physician.
Essential Functions:
Ø Obtains accurate patient histories and physical examinations to develop an appropriate assessment and safe and cost-effective plan.
Ø Appropriately consults with supervising physicians regarding clinical questions and concerns.
Ø Educates patients about current diagnoses and prescribed therapeutic regimens.
Ø Provides emotional support to patients and family members.
Ø Refers patients appropriately to outside specialists.
Ø Covers physician provider tasks when physicians are offsite.
Ø Participates in the clinical supervision and training of APP students.
Ø Functions as a member of clinical team in maintaining patient flow and maximizing the delivery of quality care to patients.
Ø Emphasizes health promotion and/or disease prevention through patient education and coordination of care.
Ø Reviews and addresses all assigned items related to documentation, tasking and results within the EHR.
Ø Completes all documentation and billing related to encounters no later than 2 business days from the date of service.
Ø Assists with tracking or documenting any quality measures relating to identified payers or programs the practice is enrolled in.
Ø Responsible for reviewing company email daily.
Ø Performs certain other assignments as determined by the Medical Director and/or Division Chief including, but not limited to, participation in departmental conferences and participation in the Quality Improvement efforts of the department.
Ø Discusses concerns with Collaborating Physician, Medical Director and/or Division Chief in a timely manner to ensure efficient and effective patient care.
Ø Follows regulations and guidelines for writing prescriptions
Minimum Job Requirements:
Education: Current NYS License as Physician Assistant or Nurse practitioner.
Experience: Minimum two years’ experience with healthcare delivery, preferably in adult primary care. We are willing to consider newer graduates.
Knowledge, Skills & Abilities: Excellent patient centered communication and attention-to-detail required. Experience with electronic health records required.
