What are the responsibilities and job description for the Case Manager, Full Time position at Buffalo Valley Inc?
SCHEDULE: 1st Shift, Monday - Friday SALARY: D.O.E
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Orients new clients as to goals and objectives of program, role of the case manager, and services that are available and accessible to the client
- Completes Initial Admission Assessments and Admission packet on all clients assigned to them within 36 hours of entry
- Assist clients in addressing issues related to A&D addiction, homelessness, job training/placement
- Maintains up-to-date, accurate and understandable case files and records, as delineated on the Chart Order Checklist
- Attends scheduled team meetings and training sessions
- Conducts follow-up on all assigned clients for a minimum of one (1) year
- Prepares monthly/quarterly reports of activities and submits to supervisor on scheduled due date
QUALIFICATIONS
- Pass Background Check and Drug Screen required
- Must have strong computer skills with ability to type 30 wpm
- Skilled in addressing the special needs of the homeless population, while displaying a compassionate and professional approach.
- Must demonstrate effective interpersonal communication skills and strong organizational skills with paperwork
- Keeps a current CPR and First Aid certifications, required
- Valid TN D.L. with "F" endorsement required; "F" endorsement can be obtained once hired
EDUCATION and/or EXPERIENCE
- Higher Learning Degree preferred but not required
- Two (2) years of related experience and/or training; or equivalent combination of education and experience as well as previous experience working in chemical dependency field preferred. Adequate understanding of clients who have A & D and/or mental health issues
- High School Diploma or GED required
